Dear Ms. Chiappone:
The following are submitted in response to your letter of June 22, 2000 regarding items that you
determined the Division must receive prior to considering the above-referenced application as complete.
APPLICATION FORM: The General Location information on Page 2, Item 9 has been entered, and the
original and four (4) copies of that page are enclosed.
EXHIBIT O -Proof of Mailing of Notice to County Commissioners and Soil Conservation District (Rule
6.4.17): Nick Malakar of Haley & Aldrich advises that the return receipt from the County was sent to you
on June 28, and the return receipt from the Soil Conservation District was mailed to you July 6.
EXHIBIT S -Permanent Made-Made Structures (Rule 6.4.19): We are working on obtaining agreements
with the owners of structures located within two hundred (200) feet of the operation or affected land, and
evidence of such agreements will be submitted to the Division upon receipt. You indicated in our
telephone conversation yesterday that this would satisfy the requirement of accepting our application as
complete.
ADDENDUM 1 -Notice Requirements -Affidavit of Posting. Notice (Sign) Rule 1.6.2(a)(b): Enclosed
is the required Certification.
In addition to the above items, as addressed in your letter, a Proof of Filing with County Clerk and
Recorder is also enclosed for insertion under EXHIBIT R.
Based upon our telephone conversation of July 6, 2000, it is my understanding [hat our application will be
considered complete upon your receipt of the above-referenced items. Assuming that Monday, July 10
would be the earliest our application will be considered as filed, [ am notifying the newspaper to publish
the firs[ notice on July 19, 2000, indicating that comments must be received by DMG by August 29.
As I indicated to you yesterday, I will be out of town until July 19. If the public notice information is
