It appears that only a few of the questions were addressed. Please read <br />the remaining comments carefully and provide complete responses. The application cannot be approved until <br />the following remaining issues are adequately addressed. Please submit a response to the following issues <br />as soon as possible, but no later than July 5, 2006 so the decision due date set for July 12, 2006 can be met. <br />If you cannot respond by July 5, 2006, please submit a written request for extension of the decision date.
