Please check one below and return this <br />form with your payment to DMO. If a CD is requested end the permit is coo large for a CD, you will be called. <br />Please allow up to 5 business days from the receipt of this signed document for your CD or DvD to be available. <br />CD ®55.00 per disk _DVD ©$20.00 per disk <br />Please check if you want a copy of the QuickView irtsuillatlon instructions. <br />Computer requirements to use the DMG document imaging CD (or DVD): <br />1. A progtstn called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the <br />first time. Future use of this or other imaging CD's does net require a new installation of the software. <br />2. The CD or DV D that you purchase must be in the D: drive. A D VD drive will read a CD or DVD. <br />3. The CD will contain fires with the .tif extension. There needs to bean image viewing application for .tif files on your computer that <br />will handle single and multi-page documents. Gn a Windows 7SP computer, the default vlewing prograrus are called "Windows Picturo <br />and Fax viewer" or "PainC' and they do not work with QuickView. The Imaging V lower that comes with pie-Windows XP computers <br />does work. DMG has detemvned that a program called °IrfanView", which can be downloaded from IrfeaVicw.ram, dots work with <br />QuickView. After downloading "IrfanView", it needs to be con'Ggured as your default .tif viewer. AMG does not support or endorse this <br />software and the liccneing and registration are the user's responsibility. There are many other programs that work as well, such as <br />"Imaging Proview" end "Imaging for Windows^. <br />[nstructions• <br />1. Insert the CD into your computer. <br />2. Allow dme for die CD to install. <br />3. Eject the CD from the computer. <br />4. Reinsert the CD into the computer. <br />5. You nre ready to view the Permit Files. <br />NOTE: A program called QuickView Autorun, which runs the QuickV (ew <br />setup progrem, has also been copied to this CD.
