Please check one below and remm this form with your payment to DMG. if a CD is requested and the petmit is too large for a CD, you will be called. V CD @ $5.00 per disk _DVD @ $20.00 per disk Please check if you want a copy of the QuickView installation instructions. Computer requirements to ose the DMG document imaging CD (or DVD): 1. A progtam called QuickView will be copied m the CD and must be installed on your computer in order to use a DMG imaging CD the lust time. Future use of this or other imaging CD's does not require a new Installation of the software. 2. The CD or DVD that you purchase must be in the D: drive. A DVD drive will read a CD or DVD. 3. The CD will contain files with the .tif extension. There needs ro be an image viewing application for .tif files on your computer that will handle single and multi-page documents. On a Windows XP computer, the default viewing progtams are called "Windows Picture and Fax Viewed' or "Paint" and they do not work with QuickView. The Imaging Viewer that comes withpre-Windows XP computers does work. DMG has determined that a program filled "IrfanV iew", which can be downloaded from IrfanView.com, does work with QuickView. After downloading `IrfanView", it needs to be configured as yom default .tif viewer DMG does not suppoR or endorse this software and the licensing and registration are the [tsar's responsibility. There are many other programs that work as well, such as "Imaging Preview" and "Imaging for Windows". _ Instrnctions• 1. Insert the CD inro your computer. 2. Allow time for the CD to install. 3. Eject the CD from the computer. 4. Reinsert the CD into the computer. 5. You are ready to view the Permit Files. NOTE: A program called QuickView Autorun, which runs the QuickView setup program, has also been copied to this CD. If the Autorrm program does NOT work, you wdl need to manually install the QuickView program. Instructions for manually installing QuickView are available upon request with dte CD or can be viewed at www.minine.state.ce.us. The Permit File document images can be viewed in the reception area of the DMG office or they can be purchased oo a CD or DVD. The CD/DVD purchase cost covers the time and labor of making the CD.
