Long <br />D rvisiun Dneao. <br />On July 23, 1993, the Division of Minerals and Geology approved the <br />Roadside and Cameo Renewal application (C-81-041). Stipulations <br />were attached to the permit document, and Stipulation number 11 has <br />yet to be addressed. Stipulation 11 reads as follows: <br />WITHIN 30 DAYS OF PERMIT ISSUANCE THE PERMITTEE SHALL <br />SUBMIT TO THE DIVISION PROOF THAT AML VIOLATION NUMBERS; <br />920209025, 92020926, 92020927, 920209034 AND KENTUCKY <br />VIOLATION NO. 8895005 ARE UNDER ADMINISTRATIVE OR <br />NDICIAL APPEAL REVIEW. IF THE VIOLATION IS APPEALED, <br />THE PERMITTEE SHALL SUBMIT TO THE DIVISION PROOF OF THE <br />RESOLUTION OF THAT APPEAL, AS SOON AS POSSIBLE AFTER <br />PERMIT ISSUANCE. <br />Please provide the required information stated in the Stipulation <br />above.
