Box 161 <br />Silt, CO 81652 <br />Re: Request for information for Exploratory Drilling <br />Dear Mr. Self: <br />Per our telephone conversation today, I am enclosing a packet of <br />forms that pertain to exploratory drilling requirements in <br />Colorado. These materials should facilitate the exploratory <br />drilling approval process. <br />Please fill out the Notice of Intent form and return it to the <br />Division for final approval. <br />Two forms, (Procedures for Coal Exploration Approval and Coal <br />Exploration Sondinq) that outline the procedures to be followed are <br />also included. Please follow these quidelines for bonding and <br />exploration approval. <br />Should your exploration operation exceed 180 days, then a Coal <br />Exploration 180 day Report form needs also to be filled out and <br />submitted to the Division. I have included one for your use if <br />necessary. <br />At the termination of exploratory drilling, the form Coal <br />Exploration Final Reclamation Report needs to filled out and <br />returned to the Division. <br />Finally, at the conclusion of all activity on the site, a Coal <br />Exploration Drill Eole Abandonment Report needs to be filed. <br />
