Box 17304 <br />Boulder, CO 80308-0304 <br />Re: Permit Adequacy Information Submitted May 2, 1995, Gold Hill Mill, <br />File No. M-94-117 <br />Dear Mr. Hartley: <br />The Division has reviewed your adequacy responses to our March 27, 1995 <br />list of concerns and issues with the Gold Hill Mill permit application. <br />There are a number of clarifications required, and certain concerns to be <br />addressed before the Division can approve the application. Please provide <br />responses to the following list of issues prior June 1, 1995 in order to <br />facilitate a June 14, 1995 permit issuance date. <br />1. In our meeting on May 16, 1995, it was indicated that the exact <br />permit boundaries to be proposed have not yet been mapped on the basis of <br />your site survey. As you know, all surface disturbances related to the <br />mill operation will have to be included on a map, including the road <br />between the mill and the tailing impoundment, all proposed borrow areas, <br />and the eventual footprint for the expanded tailing facility. The map <br />must be prepared in accordance with the general requirements of Rule <br />6.2.1(2). An accurate permit area acreage must be provided on a revised <br />application form. As a point of clarification, the road between the mill <br />and the tailings is a pre-operational feature, and there is no problem <br />with the road remaining when operations are concluded, as long as it is <br />in a stable condition, but this is not a Forest Service road. <br />2. No land ownership map was included in your May 2, 1995 submittal. <br />Please provide a land ownership map as requested in item no. 2 of the <br />Division's March 27, 1995 letter. <br />3. A copy of the milling agreement between COM, Inc. and Mi Vida <br />Enterprises will be required to establish legal right to enter those <br />portions of the proposed permit area that are not owned outright by COM, <br />Inc. A copy of the milling agreement was not included in the May 2, 1995 <br />submittal to the Division. <br />4. If COM, Inc. proposes to mine on the Oscar or the Good Enough <br />
