Photos of the cabin before and after the 5 NW longwall panel had been mined past the <br />cabin location. -The first set of photos were taken on November 3, t 994. The second <br />set of photos were taken on December 13, 1994, and the third set were taken on <br />December 20, 1994. MCC was accompanied by a Mautz family member or representative <br />when the photos were taken. Their comments were noted at that time. The poor interior <br />drywall finishing and the separation of the exterior entrance slab were observed even then. <br />Tony Mautz admitted at that time that the separation of the slab "probably would have <br />happened even without the mine underneath" as the footer and slab were not tied together <br />with rebar and there is always a certain amount of settling when a new building is built. <br />He also said that they were pleased that no subsidence damage had occurred. <br />2. A memo, prepared by a MCC Engineer with several years experience in the mid-West <br />with longwall mining subsidence effects to surface lands and structures, noting <br />observations and comments regarding the Mautz cabin. <br />3. A letter from the home building contractor that completed the recent repairs to the <br />cabin, detailing the repairs that were made and providing general comments regarding the <br />construction. <br />4. A copy of the pre-subsidence survey data and the final survey of the cabin noting the <br />cumulative subsidence from mining both the 5 NW and 6 NW longwall panels. <br />Since the above-referenced letter was sent, the Division has conducted another inspection of the <br />cabin and is aware of the status of the solutions (repairs) discussed during the first inspection. <br />
