There are many that have been written for maintenance of <br />the drainage system or the pond associated with the refuse pile. <br />These reflected short-term threats to the integrity of the pile, <br />but have been satisfactorily resolved. <br />C-92-001 Failure to follow plan for construction and maintenance <br />of refuse pile, specifically, 1) Utilization of a refuse <br />product which differs from that projected by the plan, <br />upon which stability analyses were run; 2) Failure to <br />maintain drainage off the crest of the fill. <br />Issued 1/10/92 <br />Terminated 4/9/92 Civil Penalty $1500 <br />C-91-033 Failure to perform late fall seeding of re-topsoiled <br />areas in accordance with the plan. <br />Issued 12/31/91 <br />Terminated 1/16/92 Civil Penalty $750 <br />C-91-031 Failure to maintain two sedimentation ponds: Sediment <br />Pond 1's sediment level exceeded the designed level and <br />the west end, near the inlet, is eroding excessively AND <br />the eastern emergency spillway pipe of Pond 4 (Refuse <br />Pile) was crushed on the inlet. <br />Issued 12/31/91 <br />Terminated 3/3/92 Civil Penalty $750 <br />C-91-030 Failure to provide a combination of principal and <br />emergency spillways or a single open channel spillway or <br />documentation that the pond can contain the 25 year 24- <br />hour event. (The refuse pile pond was one of seven ponds <br />cited in this violation). <br />Issued 12/31/91 <br />Terminated 4/9/92 Civil Penalty $500 <br />
