Please check one below and return this form with your payment to DMG. If a CD is requested and the permit is too large for a CD, you will be called. Please allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. ~CD Q $5.00 per disk _ DVD Q $20.00 per disk Please check if you want a copy of the QuickView installation instructions. Computer requirements to ose the DMG docameet imaging CD (or DVD): 1. A programm called QuickView will be copied to the CD sad must be installed on your oomputer im order to use a DMG imaging CD the first time. Future use of this or other imaging CD's does not require a sew installation of dre software. 2. The CD or DVD that you purchase must be is the D: drive. A DVD drive will read a CD or DVD. 3. The CD will contain files with the .rif extension. There needs to bean image viewing application for .tif files on your computer that wilt handle single and multi-page documents. On a Windows XP computer, the default viewing programs are called "Windows Picture and Fax Viewer" or "Paint" and they do not work with QuickView. The Imaging Viewer that comes withpre-Widows XP computers does wotlc. DMG bas determined that a progam called ^IrfanView^, which can be dowmloaded fiom L:fanView.com, does work with QuickView. After downloading "IrfanView^, it needs to be configured as your default .tif viewer DMG does no[ support or endorse this software and the licensing and registration are the user's responsibility. Thera are many other programs that work as well, such as °Imaging Preview^ and ^Imaging for W inflows^. Instrnctions• 1. Insert the CD into yonr computer. 2. Allow time for the CD to install. 3. Eject the CD from the computer. 4. Reinsert the CD into the computer. 5. You ate ready to view the Permit Files. NOTE: A program called QuickView Autonm, which runs the QuickView setup program, has also been copied m this CD. If the Autorun program does N07 work, you will need to manually install the QuickView program. Inshuctioas for manually installing QuickView are available upon request with the CD or can be viewed at www.minina.state.co.us. The Permit File doeament images can be viewed to the reception area of the DMG office or they can be pnrcbased on a CD or DVD. The CD/DVD purchase coat covers the tlme and labor of making the CD. DMG cannot urovide computer related suaoort.
