Long F1oreIICe, CO 81226 Division Direcmr Re; Record Compliance for Twin Pines TYin Pines No.2 (C-83-058) Dear Ruth; During my last inspection on Wednesday, July 20, 1994, it was evident that several record keeping functions have been delayed and need immediate attention. In particular, the Certificate of Insurance for the Twin Pines Mine No. 2 has expired, and the mine is in default. I did see the letter dated July 15, 1994, that Larry White provided in the record file, which indicated Twin Pines Limited Investment was in the process of exploring new quotes and that the old carrier had not been retained for the 1994-1995 year. In addition, in my brief discussion with Larry White over the records, he mentioned that the application for a Stormwater Permit was "never sent". i had addressed this issue in January 1994, (see letter attached) and was assured on many occasions by Mr. White that the application had been sent. Twin Pines Limited Investment must send the Stormwater Permit application in immediately to avoid being out of compliance with the Department of Health. Remember that at this time, the Department of Health is only requiring an application submittal, and is not reviewing plans for adequacy. The third issue, submittal of a permit renewal application, I understand, is in progress. A phone call received today from Larry White has requested another application form be sent. Enclosed is the requested permit renewal application form you requested.
