Long <br />F1oreIICe, CO 81226 Division Direcmr <br />Re; Record Compliance for Twin Pines <br />TYin Pines No.2 (C-83-058) <br />Dear Ruth; <br />During my last inspection on Wednesday, July 20, 1994, it was <br />evident that several record keeping functions have been delayed and <br />need immediate attention. <br />In particular, the Certificate of Insurance for the Twin Pines Mine <br />No. 2 has expired, and the mine is in default. I did see the <br />letter dated July 15, 1994, that Larry White provided in the record <br />file, which indicated Twin Pines Limited Investment was in the <br />process of exploring new quotes and that the old carrier had not <br />been retained for the 1994-1995 year. <br />In addition, in my brief discussion with Larry White over the <br />records, he mentioned that the application for a Stormwater Permit <br />was "never sent". i had addressed this issue in January 1994, (see <br />letter attached) and was assured on many occasions by Mr. White <br />that the application had been sent. Twin Pines Limited Investment <br />must send the Stormwater Permit application in immediately to avoid <br />being out of compliance with the Department of Health. Remember <br />that at this time, the Department of Health is only requiring an <br />application submittal, and is not reviewing plans for adequacy. <br />The third issue, submittal of a permit renewal application, I <br />understand, is in progress. A phone call received today from Larry <br />White has requested another application form be sent. Enclosed is <br />the requested permit renewal application form you requested. <br />
