Please check one below and return this form with your payment to DMG. If a CD is requested and the permit is too lazge for a CD, you will be called. Please allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. / CD @ $5.00 per disk X 3 _ DVD @ $20.00 per disk Please check if you want a~c~fpy of the QuickView installation instructions. Computer requirements to use the DMG document imaging CD (or DVD): 1. A program called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the first time. Future use of this or other imaging CD's does not require a new installation of the sof[waze. 2. The CD or DVD that you purchase must be in the D: drive. A DVD drive will read a CD or DVD. 3. The CD will contain files with the .tif exextension. There needs to be an image viewing application for .tif files on your computer that will handle single and multi-page documents. On a W indows XP computer, the default viewing programs are called "Windows Picture and Fax Viewer" or "Paint" and they do not work with QuickView. The Imaging Viewer that comes with pre-Windows XP computers does work. DMG has determined that a program called "IrfanView", which can be downloaded from IrfanView.com, does work with QuickView. After downloading "IrfanView", it needs to be configured as your default .fif viewer. DMG does not support or endorse this softwaze and the licensing and registration aze the user's responsibility. There aze many other programs that work as well, such as "Imaging Preview"-and "Imaging for Windows" - - - - Instructions: 1. Insert the CD into your computer. 2. Allow time for the CD to install. 3. Eject the CD from the computer. 4. Reinsert the CD into the computer. 5. You aze ready to view the Pemtit Files. NOTE: A program called QuickView Autorun, which runs the QuickView setup program, has also been copied to this CD. If the Autorun program does NOT work, you will need to manually install the QuickView program. Instmctions for manually installing QuickView aze available upon request with the CD or can be viewed at www.minine.state.co.us. The Permit File document images can be viewed in the reception area of the DMG office or they can be purchased on a CD or DVD. The CDJDVD purchase cost covers the time and labor of making the CD. DMG cannot provide computer related support.
