On September 26, 2006, the Mined Land Reclamation Division received your request for Financial Warranty
Reduction for the King Mountain Sand & Gravel Mine, Perrnit No. M-1978-314. We have determined that the
following information must be submitted to complete the requirements of Rule 4.14 and Rule 4.17 of the Construction
Material Rules and Regulations and C.R.S. 34-32.5-117(5) of the Colorado Land Reclamation Act for the Extraction
of Construction Materials (a copy of this Rule and Statute aze enclosed for your information):
Rule 4.17.1(21: The Operator shall included in the notice to the Office the names and addresses and phone
numbers of all owners of record to the affected land.
Rule 4.17.1(31: The written notice requesting release shall be sent by Certified Mail and be sepazate from
other types of communication to the Office.
