Dear Mr. ]ohnson:
On Febmary 2Q, 2002, the Division of Minerals and Geology received an Application for Succession of
Operator, Performance Warranty and a $125.00 fee. And on October 3, 2001, the Division received a
rider to the Liberty Mutual Insurance Company Corporate Surety Bond No. for the Colona
Pit, Permit No. M-1994-005. The Division is returning the Application for Succession of Operator and
the rider £or the following:
The Division requires that the current permittee name remain the same throughout this document,
including the notary (i.e. United Companies of Mesa County, Inc.).
The Division requires that the name on the rider be the same as the name of the Successor
Operator.
