The DMG's preliminary adequacy review letter dated January 17, 2006 requested additional information. On
behalf of CAM MINING LLC, the following amended data are provided;
1) There is incorrect address (zip code) listing for surface and mineral owner Betty L. Branch on page 2 and 3 of
permit Appendix A, apparendya typographical error. Please amend the address oflistings as appropriate.
CAM -Changed Betty Branch's zip code in Appendix A from 90210 to 80210 on page 2 & 3
2) On page 4 of Appendix A, #11 Enterpdses, Ltd., is listed as a grating leaseholder. A corresponding permit
map, Figure 1.1-3d HOLDERS OFADJACENT GRAZ/NG LEASES, lists a previous lease holder (Young) for the
East Salt Allotment. Please amend the figure as appropriate.
CAM -Changed Young to #11 Enterprises on Figure 1.1-3d. Also updated Grazing lease, no longer
Hitchborn, now Tomlinson. Amended page 4 is enclosed.
3) Figure 1-1-3a INTEREST IN SURFACE PROPERTYAND WATER R/GHTS, ident~es a parcel of land on the
East Salt Creek valley floor adjacent to (south and east ot) the permit boundary as being owned by Grand
Valley Coal Company and leased by Lodestar Energy, Inc. This information is apparentlyoutdated orincorrect,
more recent information in the Munger Canyon Mine permit documents indicate the parcel is owned by #11
Enterprises, Ltd. P/ease amend the Figure as appropriate. Outdated references to previous mine
operators should also be corrected.
CAM -Updated Figure 1.1-3a to show #11 Enterprises as current landowner (changed from Grand
Valley Coal Company). Changed McClane Ditch ownership from Young to #11 Enterprises and
changed Fee Ownership (#3) from Lodestar to CAM MINING LLC.
4) The statement on page 1 of permit Appendix 8, under paragraph (3), which responds to the requirements of
Rule 2.03.5(3), is now out of date. Please update the text of Appendix 8 to reflect the current status,
Including all such violations as described fn the rule, received by CAM during the three years prior to
the date of RN-6 application submittal. Also provide updates regarding requirements of 2.03.5(1), (2),
and the parts of (3) pertaining to listing of unabated cessation orders and unabated airand waterquality
violation notices received by CAM or othermines owned and controlled byowner/controllers of CAM(if
there have been any such suspensions, revocations, forfeitures, or unabated enforcement actions).
CAM -Updated Appendix B to reflect all NOV's received in last 3 years by CAM. Listed NOV-2005-
008, NOV-2004-005 and NOV 2005-004 (for Munger Canyon Mine). Pages 1 and 2 are enclosed.
