_~ <br />ADDENDUM REPORT <br />iii iiiiiiiiiiiii iii <br />999 <br />ABANDONMENT OF SOIL EXPLORATION DRILL HOLES <br />In wmpliance with SeMion 4.07.3 (3), the following information is provided to the Colorado Division of Mining <br />and Geology (CDMG): <br />Total number of drill holes: 22 <br />Start of Drilling: Julv 15. 1996 Completion of drilling: Julv 19. 1996 <br />Completion of Permanent Sealing: July 23. 1996 <br />Name of Drilling_Contraclor: Lambert and Associates. Montrose. Colorado <br />License Number of Drill and Rie: Unavailable <br />Tvce of CemenUSealant Used: Readi-mix Concrete (all holes) <br />Description of how holes were worked: All holes were drilled using a 4" OD continuous flight auger, which <br />was removed ceriodically to allow split spoon sampling at different <br />depths. The number of split spoon samples cer hole and the total <br />depth of drilling varied. (See Table 1). After drilling was completed. <br />each hole was backlilled by the driller with cuttings to l0 feet below <br />Rrade. Mountain Coal Comoanv emplovces came in afterwards. <br />(generally within 24 hours), mixed and poured concrete plugs to <br />within at least 3 feet below grade (a minimum 7 foot plug or minimum <br />0.6 CF of concrete per hole), and filled the remainder of the hole with <br />additional cuttings.
