I understand that you have discussed this with Allen Sorenson. <br />Enclosed for your review please find a copy of the unsigned Succession of <br />Operators Application with most of the required information inserted. Gwen Fraser will <br />sign the Applicatio.^., and then I will deliver it to you to sign at a cunve~~ient time. Please <br />note that all signatures must be notarized. COM, Inc. will provide the $100.00 <br />application fee. <br />I do not have Mi Vida's tax ID number to insert in item 4 on page 2. I will be <br />happy to add it if you can provide it to me. Also please note that page 1 provides for you <br />to initial and date the line next to Option B. Allen has advised me that there is no need to <br />waive the 30-day processing time. <br />The Division will require new Performance and Financial Warranties executed by <br />Mi Vida to replace the present joint warranties in order to approve the succession. <br />Enclosed are originals of these documents with most blanks completed.
