Pollard, K-Bar-T Pit, Permit No. M-78-221, Division Dueclo~ Succession of Operators Application Dear Mr. Proctor: On September 30, 1996, the Division of Minerals and Geology received your submittal of the Succession of Operators ("SO") application for the K-Bar-T Pit, Permit No. M-78-321. The Division requests that you submit a Power of Attorney showing that you are Ms. Pollard's legal representative and can sign legal documents on her behalf. If you cannot provide the Division with such a document the Division will have to require Ms. Pollard's signature on the Succession of Operators Application, the Financial Warranty/Certificate of Deposit, and the Performance Warranty form. Please let me know which way this will need to be handled. If we accept your signature on all the documents, your notary will need to make corrections on each document. I am returning the original So application, the Financial Warranty/Certificate of Deposit form and the Performance Warranty form to you. Ms. Samme Elder will need to make changes in her acknowledgment of your signature if we accept your signing on Ms. Pollard's behalf. Also, all the documents must be consistent as far as the operators name. Since Ms. Pollard is the Operator, her name must be listed first on the documents. Example: Alma M.
