The Division has completed the preliminary adequacy review of permit revision 04. Listed
below are the issues that require additional or revised information. Please address each
issue and submit two copies of the revised pages or maps as required.
1. Rule 2.03.4(2)(a) -Please provide the Employer ID number for the New Horizon
Mine.
2. Rule 2.03.4(3)(b) -Please provide the percentage of ownership of WFC that each
owner and controller possesses.
3. Rule 2.03.4(3) - If the owners/controllers of WFC listed on page 2.03.2 do not own
or control another coal mining operation or have applied for another coal mining
operation, it should be stated on page 2.03.4.
4. Map 2.03.4-1 indicates Ward and Alma Burbridge own a portion of the surface
property within the proposed mine boundary, however, their names are not included
in the list of surface owners. Additionally, Map 2.03.4-] needs. to include the permit
boundary.
5. Map 2.03.4-2 indicates D. Pulliam as a mineral owner but his name is not listed in
the text.
6. Rule 2.03.4(6)(b) - On page 2.03-10, Shell Oil Company is listed as a mineral owner,
however, Shell Oil is not listed on Map 2.03.4-2, nor is there a parcel identified for
the lessee. Additionally, a small parcel in parcel 24 (Salazar) has been drawn but
the ownership of this parcel is unclear.
