The name of the owner of the telephone line, Qwest Communications. <br />Exhibit D-Mining Plan <br />3. The sediment pond shown on the mining plan map will be approximately 50'wide and <br />100' long and no deeper than 5' from floor of the pond to spillway elevation. The <br />sediment pond will discharge to the southeast via grip-rapped lined spillway, no less than <br />6' across and 16' in length prior to flowing through a 36" culvert to the south side of <br />County 2. At that point natural drainage is in a northeastern du•ection The sediment <br />pond will be constructed with soils pushed up at that location. One foot of freeboard will <br />be allowed from crest of spillway to crest of berm. Since the sediment pond is being <br />located within a depression, the northeast and southwest sides of the sediment pond may <br />be graded to the existing slope rather than bermed. The spillway end of the <br />pond(southeastern end) will likely be bermed with no less than eight feet across the top of <br />the berm (and spillway) and a spillway and berm outslope of no more than 3:1. The rip- <br />rap used will be no less than 8" and no more than 24". The northwestern aspect ofthe <br />pond may be graded to the existing slope along the bottom of the existing drainage way. <br />
