The terms of the CN No. <br />001 approved by the Division are hereby incorporated into Permit No. M-80-201. <br />The conditions of the Division approval are noted below. Please note that all <br />other provisions of Permit No. M-80-201 remain in full force and effect. <br />Stipulation No. O1 <br />THE AMOUNT OF FINANCIAL WARRANTY FOR THE CONVERTED PERMIT IS SET BY THE <br />DIVISION AT $12,000.00. MATHIAS CONCRETE MUST SUBMIT A NEW BOND, OR <br />+1DJUST THE EXISTING BOND TO THIS AMOUNT. <br />Stipulation No. 02 <br />OPERATOR WILL NOT EXCEED 8 ACRES IN MAJOR DISTURBANCE, 1.2 ACRES <br />INMODERATE DISTURBANCE, AND 4.8 ACRES IN MINOR DISTURBANCE AT ANY POINT <br />DURING THE LIFE OF THE MINE. <br />Please find enclosed a copy of correspondence received by the Division from <br />the Army Corps of Engineers regarding this pit. <br />If you have any questions, please contact me. <br />Sincerely, <br />Allen C.
