This minor revision pertains to updated haul road as-built certifications. These changes were made to reflect the actual conditions which currently exist for the various haul roads which have been modified since their last certification. This revision addresses TDN x95-020179-002, Violation #3 which was issued to the Division by OSM during their oversight inspection on March 28 - 31, 1995. The following revisions were made to the specified exhibit: Exhibit 13-11.1: - Riprap added to the outlet of culvert TAHRC-1 Exhibit 13-11.3: - Riprap and silt fence placed in ditch TAHRD-7 Exhibit 13-12.2: - Silt fence located at the outlet of culvert ME-5 - Name change to ditch HRD-A2ME5 Exhibit 13-12.3: - Location of culvert F-3 - Name change to ditch HRD-A2ME5 Exhibit 13-13.1: - Riprap placed in ditch HRD-A1 - Name change to ditch HRD-A2ME5 - Flow of HRD-A2ME5 bypassing Pond 006 Exhibit 13-13.2: - Silt fence located at the outlet of culvert A-3 - Movement of permit boundary to proper location Exhibit 13-13.3: - Silt fence located at the outlet of ditch HRD-AS Enclosed are the required application form, insertion instructions into the PAP, and the revised exhibits.
