This is a report of findings and a request for information as a result of an internal review of the
Colorado Coal Mine No. 1, G81-024 Applicant Violator System (AVS) databank and internal files.
In this memo, requests for documentation means copies of letters of appointment, letters of
resignation, minutes of board meetings, contracts, etc. However, certificates from corporate
seeretazies that cite specific sources have been accepted.
Enclosure 1 is a schematic of organizations that "owns or controls" [Rule 2.03.4(3)] the applicant
according to information in the AVS databank. Enclosure 2 is information regarding Pernlit C-81-
024. Enclosure 3 is information regarding Kaiser Ventures Inc. Enclosure 4 is information
regarding Pension Benefit Guarantee Co. Enclosure 5 is information regarding Volunteer
Employees Benefit. The information in these enclosures should match that of the Permit [Rule
2.03.4(3)].
With regard to Enclosure 2, note that the begin dates for Kaiser Resources Inc as Operator and Mike
Keegan as agent are missing.. Please provide me with documentation for these dates for entry
into the AVS databank, and any other changes you find appropriate.
