Catfany <br />Division Director <br />Natural Resource Trustee <br />On April 25, 2006, the Division of Minerals and Geology received First Bank of Longmont Irrevocable Letter of <br />Credit No. 864-9189, in the amount of $12,500.00, and a Financial Warranty Letter of Credit form, for Purdue, Permit <br />No. M-2000-089. The Division is returning this document for the following: <br />The Division requires that the Irrevocable Letter of Credit be submitted on bank letterhead and in language <br />approved by the Boazd. Please refer to the enclosed sample Irrevocable Letter of Credit for appropriate <br />language. <br />The Division requires that the Operation name listed on the Irrevocable Letter of Credit and Financial <br />Warranty Letter of Credit form match Operation name listed on the permit. Our records show the Operation <br />name is Perdue and the Operation name listed on the Irrevocable Letter of Credit and the Financial Warranty <br />Letter of Credit form is Ignacio A. Vasquez, Jr. and Armando Vasquez.
