Cattany <br />Division Director <br />Natural Resource Trustee <br />Re: Brownstone Quarry, File No. M-1997-065, Receipt of 112 Construction Materials Reclamation Permit <br />Conversion Application Package (CN-Ol) <br />Deaz Mr. Striggow: <br />On April 19, 2007 the Division of Reclamation, Mining and Safety received your 112 Construction Materials <br />Reclamation Permit Application package for the Brownstone Quarry, File No. M-1997-065. Preliminary review of the <br />information received determined that the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPLICATION FORM: <br />o Stone Wholesale Corporation is not listed with the Colorado Secretary of States Office (SOS) and <br />therefore not an entity registered to do business in the State of Colorado. However, Stone Wholesale, <br />Inc. is listed with the SOS, and therefore an entity that can do business in the State. The applicant will <br />need to either register Stone Wholesale Corporation with the SOS or transfer the existing permit from <br />Stone Wholesale Corporation to Stone Wholesale, Inc. A copy of a Transfer of Permit and Succession <br />of Operators from can be obtained from the Division's web page at http://minin¢.state.co.us. Any <br />changes to the applicant name will need to appeaz on the application form. <br />o On page 2 of the Application Form (#10), please include the Primary Mine Entrance Location in either <br />Latitude/Longitude or Universal Transverse Mercator (UTM) coordinates. <br />o On page 3 of the Application From (#11), please include the zip code for all of the correspondence <br />information. <br />o The signature page (page 8) must be signed exactly as listed on the application form (i.e. Stone <br />Wholesale Corporation).
