Box 310 <br />San Luis, CO 81152-0310 <br />RE: Phase II Raise I of Tailings Embankment <br />Dear Mr. Kluksdahl: <br />~,~` , ~ ° S <br />9 <br />4 e ~ lJ. <br />N !• <br />On August 20, 1993, the Division received your proposa:_ for start: <br />of construction on the first raise of the tailings dam. <br />Your proposal is basically as you, Alana Scott, Allen ;6orenson <br />and I discussed by phone earlier in the day August 20t1L. As I <br />stated to you on the phone, your proposal is approved <Lnd you ma}~ <br />start earthwork on the dam raise in accordance with your proposa]- <br />with the following clarifications. <br />1. The consolidation monitor stations will be in the form of <br />consolidation plates with attached stems for surveying <br />purposes. The plates must be at least 3 feet square and <br />made of 1/4 inch plate steel. The stems will be at least 2 <br />inch diameter steel pipe welded to the center of t:he plate <br />and long enough to provide a surveying surface after <br />placement of the first 10 feet of the raise. This design is <br />sufficient for consolidated drained tailings. Contact our <br />office for specifications for consolidation platers to be <br />placed on saturated tailings or slimes. If you have another <br />design for the consolidation monitor stations, pleease have <br />it approved by our office prior to placement. <br />2. The consolidation plates will be surveyed daily during <br />placement of the fill and afterwards until consolidation hay; <br />levelled off to the satisfaction of the Division. <br />3. The two additional consolidation plates will be placed <br />directly under what would be the upstream edge of the crest <br />of a second 10 foot lift and 1/3 and 2/3 of the way across <br />the length of the first 10 foot lift. <br />
