And on October 3, 2001, the Division received a <br />rider to the Liberty Mutual Insurance Company Corporate Surety Bond No. 14-006-823, for the <br />United/Norwood Pit, Permit No. M-1988-037. The Division is retLUning the Application for Succession <br />of Operator and the rider for the following: <br />The Division requites that the current permittee name remain the same throughout this document, <br />including the notary (i.e. United Companies of Mesa County, Inc.). <br />The Division does not hold a Liberty Mutual Insurance Company Corporate Surety Bond No. 14- <br />006-823 for the site.
