Re: Western Slope Flagstone Quarry No. 2, Permit No. M-1996-076
Dear Ms. Ericksen,
I tried several times to contact you about your review of the above referenced project. I was
referred toTanya and left a message for her to contact me about the items of concern. I have
not heard from her.
The following is a response to your letter of March 7, 2002 which indicated you needed several
items before technical review can begin.
APPLICATION FORM: Page 1, Item 1: The Division require that the ApplicanUOperator
name be the same as the name on the permit.
Response: According to the records, the Applicant/Operator name is the same. The
company name of Westem Slope Flagstone was added to the application. Please be more
specific.
Page 3, Correspondence Information: The Division requires that the company name be the
same as the name on the permit.
Response: The pit name of CAMEO GRAVEL has been crossed through and replaced with
the company name of Western Slope Flagstone which is what we think you wanted to be
changed. Please be more specific.
Page 7, Signature Page: The Division required That the application form be signed in the
presence of a Notary Public by the Permittee.
Response: Both applications submitted to the Division were signed by Rudolph Fontanari in
the presence of a notary. Westem Slope Flagstone was added as a company name and
revised on the acknowledgment statement. The revised original Ivory (112) application is
being returned along with a copy of the (110) application.
