On June 27, 2003, the Division of Minerals and Geology received your the I10 Construction Materials
Reclamation Permit Amendmerrt Application package for the Rock Island Operation, Permit No. M-1998-052.
Preliminary review of the information received determined that the following items must be received before the
Division can consider your application as being submitted and technical review can begin:

indicating the date on which the application was placed with the local Coumy Clerk and Recorder.
Also, refer to Rule 1.6.2(1)(c). The document that was submitted for this exhibit states that "A copy
of the Limited Impact Operation (110) Reclamation Permit App6cati9on Form (without the
exhibits) is attached for your information". You must file a complete application including all
exhibits and maps with the County Clerk and Recorder. The receipt that you will need to submit to the
Division should state that the entire application and exhibits were placed with the County Clerk and
Recorder for public review.

sufficient to demonstrate that the stability of any strctures located within two hundred (200) feet of the
operation or affected land will not be adversely affected. If the Office determines that such information
is inadequate to demonstrate that the operation will not adversely affect the stabihry of any significant,
valuable or permanent man-made structure, the Applicant shall either:
(a) provide a notarized agreement between the applicant and the person(s) having an
interest in the structure, that the applicant is to provide compensation for any damage to the structure;
or
(b) where such an agreemem can not be reached, the applicant shall provide an appropriate
engineering evaluation that demonstrates that such structure shall not be damaged by
activities ocarrring at the mining operation.
