Cattany <br />Palisade, CO 81526 Division Director <br />Natural Resource Trustee <br />/ i <br />Re: Rock Island Operation, Permit No. M-1998-052, Receipt of 110 Construction Materials <br />Reclamation Permit Amendment Application Package, Incomplete Notice, AM - i f/ <br />Deaz Mr. Brown: <br />On June 27, 2003, the Division of Minerals and Geology received your the I10 Construction Materials <br />Reclamation Permit Amendmerrt Application package for the Rock Island Operation, Permit No. M-1998-052. <br />Preliminary review of the information received determined that the following items must be received before the <br />Division can consider your application as being submitted and technical review can begin: <br />F,XHiRiT i _ Prnnf of Filino with (nnnfir Clerk (Rnle 6.i 91~ PiOVide an affidaVlt Ot tecelpt <br />indicating the date on which the application was placed with the local Coumy Clerk and Recorder. <br />Also, refer to Rule 1.6.2(1)(c). The document that was submitted for this exhibit states that "A copy <br />of the Limited Impact Operation (110) Reclamation Permit App6cati9on Form (without the <br />exhibits) is attached for your information". You must file a complete application including all <br />exhibits and maps with the County Clerk and Recorder. The receipt that you will need to submit to the <br />Division should state that the entire application and exhibits were placed with the County Clerk and <br />Recorder for public review. <br />F.XH1RiT T. _ Permanent Mnn_Madr RtrnrNmn•Pa ($~ [P~ (._i.12,); You must provide information <br />sufficient to demonstrate that the stability of any strctures located within two hundred (200) feet of the <br />operation or affected land will not be adversely affected. If the Office determines that such information <br />is inadequate to demonstrate that the operation will not adversely affect the stabihry of any significant, <br />valuable or permanent man-made structure, the Applicant shall either: <br />(a) provide a notarized agreement between the applicant and the person(s) having an <br />interest in the structure, that the applicant is to provide compensation for any damage to the structure; <br />or <br />(b) where such an agreemem can not be reached, the applicant shall provide an appropriate <br />engineering evaluation that demonstrates that such structure shall not be damaged by <br />activities ocarrring at the mining operation. <br />Office of Office of Colorado <br />Mined Land Reclamation Active and Inactive Mines Geological Survey <br />
