Request, SL-1/
Deaz Mr. Ewegen:
On May 18, 2004, the Mined Land Reclamation Division received your request for full reclamation responsibility release
for the 124th Estates Sand and Gravel Mine, Permit No. M-2001-085. Preliminary review of your request determined
that the following information must be submitted to complete the requirements of Rule 4.16 of the Construction Material
Rules and Regulations and C.R.S. 34-32.5-117(5) of the Colorado Land Reclamation Act for the Extraction of
Construction Materials:
4.16.1(2) The Operator shall include in the notice to the Office the names and addresses and phone numbers of
al] owners of record to the affected land.
4.16.1(3) The written notice requesting release shall be sent by Certified Mail and be separate from other types
of communication to the Office.
4.16.1(4) Such notice shall contain a signed statement by the Operator or their agent that all applicable portions
of the Reclamation Plan requirements have been satisfied in accordance with these Rules and all applicable
requirements under the Act.
