(BMRI) -San Luis ~H /~ <br />DMG Permit No. M-88-1 12 <br />Dear Mr. Dillie: <br />This letter is in response to your letter dated May 21, 1996. The letter requests that ISMRI, prior to <br />ilie board meeting, submit a plan that will outline specific protective measures tc assure, to <br />the satisfaction of the Division, that the tailings facility containment system will no' be <br />compromised or breached by any leak in the tailings disposal pipeline (especiall•i, where the <br />geosynthetic liner is exposed. As stated in BMRI's letter dated April 17, 1996, a potion of <br />conveyor material has been placed underneath the flange. This material is expected to be <br />more durable than steel under the same circumstances. <br />It is obvious that BMRI's procedures of routinely checking the tailings pond works to our <br />advantage. The leaking flange had been inspected by a Mill Foreman at 5:00 A Vi and then <br />again by the Mill General Foreman at 6:30 AM, less than 1.5 hours since the liner f piled. In <br />addition io these routine inspections, an additional item will be placed onto the weekly <br />maintenance schedule of the mill maintenance crew. Once per week someone will torque <br />the bolls on each flange of the line that is running. There can be as many as 23 f anges <br />running at a given time, but more likely to be about 10. The original leak was caused by ilie <br />bolts on the flanges becoming loose, allowing material to escape through a small opening at <br />high velocity and eventually cutting at the steel. If these bolts are kept tight on a regular basis, <br />this particular problem should not occur again. <br />Sincerely, <br />Sally H.
