Thor sm: <br />Section 34-32-116(3) and (5), C.R.S. (1989 Supp.) and Permit No. M-81-020 <br />require you to submit a $50.00 annual fee and an annual report on or before <br />May 6 of each year, for the previous years operations, until your rec emotion <br />bond is released in its entirety. As of the date of this letter we have not <br />received your annual fee and report. Therefore, you are in violation of the <br />above cited statute and the provisions of your Permit No. M-81-020. Pursuant <br />th Section 34-32-124(7), C.R.S.
