Proof of publication and return receipts for neighboring land owner notices were <br />completed in January, 2006. However, the inclusion of Forest Service Road 228 <br />into the proposed affected land boundaries has prompted a re-publication as well <br />as re-filing of notices to those property owners within 200 of tbe min site <br />affected lands and additional land owners bordering FS Road 228 and the Lilly <br />Mill Site(Staging Area) A proof of publication of public notice circulated in a <br />local newspaper, and all owners of record of surface and mineral lands, easement <br />holders, and owners of record of surface lands that are within 200 feet of the <br />boundary of the newly proposed affected lands will be sent to your attention as <br />soon as the return receipts are available. <br />2. All maps submitted to DMG will be signed by a qualified individual. <br />3. Colorado Marble, LLC, the original name listed as peffiit holder, has the <br />necessary succession of operator paperwork for name change to Colorado Mazble, <br />Inc., including surety bonding criteria, that will be sent directly to DMG offices <br />from corporate offices in Cartersville, Georgia <br />4. The amended affected land boundazy map included in this maili~ will show all <br />significant, valuable, and permanent man-made structures including roads, <br />ditches, pipelenes, gas lines, gas wells, utility lines, water wells and historic <br />mining stnrctures adjacent to the mining area, roadway, and stagier area. <br />
