The reasons for requesting the <br />extension are: 1) The Board Order was mailed to the wrong address <br />and the operator did not receive the Board Order until <br />approximately 30 days after the initial mailing; and 2) The <br />operator assumed after talking to staff members other than the lead <br />specialist who were not aware of the compliance deadline that <br />substantial additional time was available to complete the review of <br />the amendment application. Therefore, Division staff requests an <br />extension to June 22. 1994 for approval of the amendment. <br />
