Walther <br />Executive Director <br />Division Director <br />Natural Resource Tmstee <br />On September 2, 2003, the Division of Minerals and Geology received your 110 to 112 Construction Materials <br />Reclamation Permit Conversion Application package for the Deakins Pit, Permit No. M-2001-060. <br />Preliminary review of the information received determined that the following items must be received before the <br />Division can consider your application as being submitted and technical review can begin: <br />APPLif ATTnN FnRM - PnovP 1 _ ifam 3 - PermrHPd ArrPno~('nPa. nr P1nc6og CitPr 'j'j1e <br />permitted acreage listed on the application is indicated to be 10 acres. Please correct this amount to <br />9.99 acres and show the total acreage for item 3.2. Please be sure to change this information on the <br />application on file at the Moffat County Clerk and Recorder and submit a receipt from the County <br />Clerk and Recorder stating that changes were made to the application. <br />(3) You must provide an estimate of the project water requirements including flow rates and <br />annual volumes for the development, mining and reclamation phases of the project. <br />(4) You must indicate the projected amo7urt from each of the sources of water to supply the project <br />water requirements for the mining operation and reclamation. <br />(5) You must ailirmatively state that the Operator/Applicatrt has acquired (or has applied for) a <br />National Pollutant Discharge Elimination System (NPDES) permit from the Water Quality Control <br />Division at the Colorado Department of Health, if necessary. <br />F.XHiRTT C - PPrmanenr Mnn_MadP fitrnrfirrea t~RnIP h.d.191~ YOU must provide information <br />sufficient to demonstrate that the stability of any structures located within two hundred (200) feet of the <br />operation or affected land will not be adversely affected. You must either provide information <br />sufficient to demonstrate that the stability of First Street will not be adversely affected or an agreement <br />with the owner of First Street.
