Dear Mr. Ferguson:
On June 19, 200b, The Division of Minerals and Geology received a Transfer of Permit and Succession of Operator
Application Form, Performance Wan•anty, and a $125.00 fee for the Evans #2 Pit, Permit No. M-2000-041. The
Division is returning these document for the following:
Succession Of Operator Application:
The Division requires receipt of a Financial Warranty with the submittal of Transfer of Permit and Succession
of Operator Application when Option B has been selected.
The Notary for Successor on page 5 needs correction. The Successor Operator's title and company name
should be listed.
Performance Warranty:
The Notarization of Operator's Acknowledgement section on page 3 needs correcting. The Operator's title and
company name should be listed
