<br />This memo shall follow-up on my earlier memos dated November 5, 1984 and
<br />December 6, 1984, regarding Notice of Violations (NOV's) and pattern of
<br />violation. Since the writing of my December 6, 1984 memo, the following
<br />events have taken place:
<br />NOV C-84-157 - Sent to collection agency on December 19, 1984.
<br />NOV C-84-164 - Terminated January 29, 1985, assessment conference held
<br />January 10, 1985, penalty of $2,200.00 was fixed
<br />February 26, 1985, has not been paid. Administrative process
<br />is complete.
<br />NOV C-84-165 - Vacated January 30, 1985, as a result of January 10, 1985
<br />assessment conference.
<br />CO C-84-174 - Issued December 18, 1984, failure to abate NOV C-84-]64.
<br />Administrative process is complete. Terminated January 22,
<br />1985. Penalty of $9,000.00 (12 days x $750.00) was fixed
<br />February 22, 1985, has not been paid. No assessment
<br />conferences were requested.
<br />NOV C-84-175 - Issued December 18, 1984, failure to certify haul road,
<br />administrative process completed. Terminated
<br />January 10, 1985. Fixed penalty of $950.00 including $750.00
<br />assessment of fault (maximum negligence) has not been paid. No
<br />assessment conference was requested.
<br />NOV C-84-176 - Issued December 18, 1984, failure to submit bond for a
<br />technical revision, administrative process complete, penalty of
<br />$950.00 includes $500.00 for fault, was fixed
<br />February 22, 1985, has not been paid. Has not been abated, see
<br />CO C-85-023. No assessment conferences were requested.
<br />NOV C-84-177 -Issued December 18, 1984, failure to submit bond for a
<br />stipulation, administrative review complete, penalty of $950.00
<br />includes $500.00 for fault, has not been paid. Has not been
<br />abated, see CO C-85-024. No assessment conferences were
<br />requested.
