You are hereby notified, pursuant to C.R.S. 34-33-123(8)(b), that the Mined Land
Reclamation Division ("the Division") of the State or Colorado, Department or
natural Resources, proposes to assess a civil penalty against you in the amount of
$ 22,500.00
If you wisn to request a conference at which the proposed penalty may oe reviewed,
you may do so oy so inaicating on ana returning to the Division the attached copy
of this notice. 'four request should oe completed in the space provided and
returned t0 the Division within ten days after your receipt of this notice.
If you fail within ten days after service of this notice upon you to request such
conrerence, the Division will order the penalty fixed at whatever amount it deems
appropriate based upon the factors referred to in C.R.S. 1973, 34-33-123(8)(4).
