Preliminary review of the information received detemmined that the following items must be <br />received before the Division can consider your application as being submitted and technical review can begin: <br />APPi.i('ATi()N FnRM _ Page 7. The Division cannot accept the statement that the operator will not <br />adversely affect the stability of any significant, valuable or pennaneFrt man-made structures. The operator must <br />either provide information sufficie~ to demonstrate that the stability of any structures will not be adversely <br />affected or an agreement with the owners of the structures. The Division can accept a signed notarized <br />statement from the operator stating that they will stay at least 200 feet away from the structures u~.il an <br />agreement is submitted to the Division. <br />Your application will not be considered submitted until the information listed above is received and found sufficiemrt to <br />begin our review. A decision date will be established 90 days from the date of rceeipt of all of the requested information. <br />Additionally, if you have already published notice you will raced to republish notice, after the Division considers the <br />application submitted. This notice must be published once a week for four (4) consecutive weeks, starting within ten <br />(10) days of the date your application is considered submitted. We will notify you when you should initiate <br />republication of your notice.
