• iii iiiiiiiiiiiiiiii • <br />_2_ <br />NOTICE OF VIOLATION <br />Based upon these same Findings of Fact and in accordance with <br />C.R.S. 34-32-124(6), the Colorado Mined Land Reclamation Board hereby finds <br />Frontier Materials, Inc. to be in violation of Permit M-76-038, and C.k.S. <br />34-32-123(2). <br />Based upon these same Findings of Fact and Notice of Violation, and in <br />accordance with C.R.S. 34-32-124(2),(6) and (7) the Colorado Mined Land <br />Reclamation Board hereby orders Frontier Materials, Inc. to: <br />1. Complete the following corrective action: <br />A. File a complete amendment application to the Division which includes <br />the area of disturbance outside the permit boundary by May 26, 1992. <br />2. Pay the Colorado Mined Land Reclamation Board a civil penalty in the <br />amount of $700.00, within 30 days of the signature date of this document. <br />This is based upon an assessment of $200.00 per day for 14 days of days of <br />violation, from April 9, 1992 to April 23, 1992, with $2,100.00 suspended upon <br />completion of the corrective action outlined in this order. <br />
