Following are the <br />Division's comments on your responses. <br />l.a. Enclosed Certified Board Resolution: At the top and in the board resolution itself Central <br />Appalachia Mining, LLC (CAM, LLC) is named Central Appalachian Mining, LLC. Please have <br />the board resolution fixed to read Central Appalachia Mining, LLC since that is the company's <br />proper name. The Date at the top should be filled in with the date of the boazd meeting or the date <br />when the written action was done. Please submit two original signed and sealed board resolutions <br />to the Division. If there is no seal, please submit a memo stating that CAM Holdings, LLC does <br />not have a seal. <br />l.b. Since CAM, LLC has a new parent CAM Holdings, LLC please submit documentation showing <br />the officers, directors (members) and owners to CAM Holdings, LLC along with documentation <br />showing the ownership change to CAM, LLC. This documentation should include a Certificate <br />from the State where CAM Holdings, LLC was incorporated. Also Appendix A should be updated <br />to include CAM Holdings and its officers and directors. Once the Division has received this <br />information then Applicant Violator System (AVS) will be updated again, and the Division can <br />complete the AVS evaluation. <br />2.
