Long <br />, Division Doecto~ <br />Re: Jones Pit East (fornoerly Jones East Pit), Permit No. M-1987-009, Receipt of I10 to 112 Construction <br />Materials Reclamation Permit Conversion Application Package, CN-1 <br />Dear Mr. Dorenkamp: <br />On August 6, 2001, the Division of Minerals and Geology received yow 110 to 112 Construction Materials <br />Reclamation Permit Conversion Apphcatian package for the Zones Pit East, Permit No. M-1987-009. The <br />prelWninary re~~ew of the information received in the Conversion Application determined that the following items <br />must be receivr;d or addressed before the Division can consider yow application as being submitted and technical <br />review can begin: <br />ACREAGE REDUCTION/RELEASE REQUEST: Please be advised that the request to delete the 5.75 <br />acres from the east side of the permit must be submitted as a separate request for acreage reduction and <br />processed as a partial release request following Rule 4.Ib and C.R.S. 34-32.5-117(5)(a-d). A copy of the <br />Rule arrd Statute are enclosed for your information. <br />APPLICATION FORM: Please complete Item No. 12 and 13 oa Page 2 enclosed. <br />EXHDIIT M -Other Permits and Licenses (Rule 6.4.13): Please state what other permits, licenses and <br />approv;rls Prowers County holds or will be seeking in order to conduct the proposed mining and reclamation <br />operations: such as effluent discharge permits, air quality emissions pemlits, radioactive source material <br />licenses, the State Historic Preservation Office clearance, disposal of dredge and fill material (404) permits, <br />permit 'to construct a dam, well permits, highway access permits, U.S. Forest Service permits, Bureau of <br />Land Management permits, county zoning and land use permits, and city zoning and land use permits. <br />EXHIItIT S - Yermanent Man-Made Structures (Rule 6.4.19): You must provide information sufficient <br />to demcrostrate that the stability of any structures located within two hundred (200) feet of the operation or <br />affected land will not be adversely affected. Please tell the Division who owns the fence that divides the <br />present area of mining from the area that will be eventually deleted from the current permit. <br />Your application will not be considered submitted until the information listed above is received and found sufficient <br />to begin ow rer~ew. A decision date will be established 90 days from the date of receipt of all of the requested <br />information. Additionally, if you have already published notice you will need to republish notice, after the Division <br />considers the application submitted. This notice must be published once a week for four (4) consecutive weeks, <br />starting within ten (10) days of the date your application is considered submitted. We will notify you when you <br />should initiate npublication of your notice. The final date for receiving comments is the 20th day aRer the fourth <br />publication or the next regular business day. <br />
