Long , Division Doecto~ Re: Jones Pit East (fornoerly Jones East Pit), Permit No. M-1987-009, Receipt of I10 to 112 Construction Materials Reclamation Permit Conversion Application Package, CN-1 Dear Mr. Dorenkamp: On August 6, 2001, the Division of Minerals and Geology received yow 110 to 112 Construction Materials Reclamation Permit Conversion Apphcatian package for the Zones Pit East, Permit No. M-1987-009. The prelWninary re~~ew of the information received in the Conversion Application determined that the following items must be receivr;d or addressed before the Division can consider yow application as being submitted and technical review can begin: ACREAGE REDUCTION/RELEASE REQUEST: Please be advised that the request to delete the 5.75 acres from the east side of the permit must be submitted as a separate request for acreage reduction and processed as a partial release request following Rule 4.Ib and C.R.S. 34-32.5-117(5)(a-d). A copy of the Rule arrd Statute are enclosed for your information. APPLICATION FORM: Please complete Item No. 12 and 13 oa Page 2 enclosed. EXHDIIT M -Other Permits and Licenses (Rule 6.4.13): Please state what other permits, licenses and approv;rls Prowers County holds or will be seeking in order to conduct the proposed mining and reclamation operations: such as effluent discharge permits, air quality emissions pemlits, radioactive source material licenses, the State Historic Preservation Office clearance, disposal of dredge and fill material (404) permits, permit 'to construct a dam, well permits, highway access permits, U.S. Forest Service permits, Bureau of Land Management permits, county zoning and land use permits, and city zoning and land use permits. EXHIItIT S - Yermanent Man-Made Structures (Rule 6.4.19): You must provide information sufficient to demcrostrate that the stability of any structures located within two hundred (200) feet of the operation or affected land will not be adversely affected. Please tell the Division who owns the fence that divides the present area of mining from the area that will be eventually deleted from the current permit. Your application will not be considered submitted until the information listed above is received and found sufficient to begin ow rer~ew. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if you have already published notice you will need to republish notice, after the Division considers the application submitted. This notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date your application is considered submitted. We will notify you when you should initiate npublication of your notice. The final date for receiving comments is the 20th day aRer the fourth publication or the next regular business day.
