An officer of your <br />company needs to sign both copies and have them notarized. One original must <br />be returned to the Division at the above address. The other original must be <br />kept on file at the mine office. <br />The company should be aware that issuance of the permit renewal may initiate <br />the time frames of enclosed stipulations to the permit.
