RE: Railhead Gravel Pit, Permit No. M- 1995-036, Succession of Operators, SO-Ol
Dear Mr, Johnson:
On Febrnary 20, 2002, the Division of Minerals and Geology received an Application for Succession of
Operator, Performance Warranty and a $125.00 fee. And on October 3, 2001, the Division received a
rider to the Liberty Mutual Insurance Company Corporate Surety Bond No. 14-005-813, for the Railhead
Gravel Pit, Permit No. M-1995-036. The Division is returning the Application for Succession of
Operator and the rider for the following:
The Division requires that the current permittee name remain the same throughout this document,
including the notary (i.e. United Companies of Mesa Cou~y, Inc.).
The Division requires that the name on the rider be the same as the name ofthe Successor
Operator.
