As such, the permit renewal <br />application must be received by our office on or before January 5, 1988, as <br />described in Rule 2.08.5 of the regulations, Enclosed is a permit renewal <br />application farm which will need to be filled out and submitted along with the <br />renewal application. <br />The minimum requirements for renewal applications are described in Rule <br />2.08.5(2) of the regulations.
