<br />iii~iiiiiiiiiiii iii <br />MEMO <br />March 31. 1999 <br />To: L Wade <br />From: R Gubka <br />CC: J King <br />RE: Overburden Blast # 412 Incident <br />A blasting related property damage incident occutred on March 30,1999 at ] 2:15 ant from what was <br />suppose to be a route overburden blast. Blast t! 4 ] 2 was scheduled to be loaded and detonated near <br />the south end of the pit by Dyno-NobelBurt Explosives crew on the stated date. Dyno-Nobel/Burt <br />Explosives Company has been ~VF'C's full time blasting contractor since ] 993. The blasting crew <br />was the regular crew, consisting of Cliff Hammond (Colorado certified blaster), Mike Hilton <br />(Colorado certified blaster), Melvin Miller (blasting technician) and Alvin Lewis (blasting <br />technician). All of the blast crew were qualified and were very familiar with the operation. concerns <br />of safeq~ and procedures. John King was the designated blasting engineer and was responsible for <br />coordinating between Dyno-Nobel and WFC. He is also responsible for setting out the seismograph <br />recorders, filling out the paper work and video taping the event. I usually take a very active role itt <br />blasting since 1 have been on site since 1993 and have delt with almost all of the blastine events at <br />New Horizon Mine. Pan of my job is to oversee the blasting engineer to assure we are following <br />good engineering blasting practices and to share my experience and knowledge with him. I also <br />serve as the backup for the blasting engineer and shifr-supervisor as needed. Lance Wade is the <br />Mine Manager and was the acting shift-supervisor during this blast. Lance has been at New Horizon <br />since the startup of the operation and has many years of blasting experience. Blasting is very <br />important to us at the New Horizon Mine and all details are discussed thoroughly before. dttrin~ and <br />after each blast. <br />Further pre-blast details pertaining to this incident: <br />a) Site: The drill site was located in the New Horizon Mine Permit boundary of Section 6. <br />The south limit of the blast was approximately 450 feet from [he south permit houndm~v and <br />due north of the Nucla Sanitation District sewer pond/facilities. The drill pattern extend 130 <br />feet to the north and 115 feet to the west. Basically a square block of ground.
