Lonti <br />Division Director <br />On March 21, 1994, you reported by phone that an air actuated bypass valve diverted the <br />plant waste slurry to the lower tailings pond, bypassing the INCO cyanide destruction circuit. <br />The cyanide levels you reported for the lower pond (19.57 mg/1 WAD and 20.45 mg/1 total) <br />automatically inquired that you initiate 24 hour security, which you stated was started on <br />March 21, 1994. Our letter to you dated January 11, 1993 states that you must continue the <br />security until the Division approves cessation. <br />I asked you for an incident report and you agreed to prepare one. Please include the <br />following information: <br />1. What caused the slurry to bypass the INCO system? <br />2. When did the slurry begin to bypass the INCO system? <br />3. When did the INCO system resume treating the slurry properly? <br />4. What volume of slurry bypassed the treatment system? <br />5. What was the concentration of cyanide in the slurry, WAD and total? <br />6. What was the estimated volume of water in the lower pond on March 21, 1994? <br />7. What will be done to prevent this from happening again? <br />
