Review of the application indicated that the gravel mining operation would be expanded to include additional lands. This expansion may deplete additional water not contemplated by the present substitute supply plan, and/or be located within 600 feet of existing wells not covered by waivers of claim to injury. Since we have not yet reversed the denial of your permit, we propose the following course of action to include the expansion in the gravel pit well permit application and the substitute supply plan: Complete the enclosed Gravel Pit Well Permit Application if there are any changes from the original application. I have written the receipt number on the form so that the $60.00 filing fee is not necessary. If there are no changes, we can use the original application, which is on file in this office. 2. Submit an Amended Substitute Supply Plan with the $217.00 fee required for renewals, since the present plan expires on October 31, 1995. The plan must include the expansion and provide replacement water for any additional water consumption. The area surrounding the expansion must be investigated to locate any wells within 600 feet of the gravel pit, and the well locations and owners names and addresses must be included with the plan. A waiver of claim to injury must be signed by all well owners who own wells within 600 feet of the gravel pit, If this is not done, a hearing will be scheduled to determine if the vested rights of others will be materially injured as a result of the gravel pit operation.
