On September 1, 2006, the Division of Reclamation, Mining and Safety received a Transfer of Permit and Succession of
Operators Application, a Performance Warranty, and a $100.00 application fee for Phil Sheridan Lode, Permit No. M-
1998-081. The Division is retluning the Transfer of Permit and Succession of Operators Application and the Perfomlance
Warranty for the following:
Succession of Operator Application:
The Division requires the company name listed for the Successor Operator be listed with the Colorado Secretary
of State's office. Phil Sheridan Lode Mine is not currently listed with the Colorado Secretary of State's office. If
you intend for Harry Covey to be the Successor Operator as an individual, please delete "Phil Sheridan Lode
Mine" from the "Company Name" blank under the Successor Operator section on page 2.
The Division requires the company name listed in the "Notary for Successor" section on page 5 be listed with the
Colorado Secretary of State's office. Phil Sheridan Lode Mining is not currently listed with the Colorado
Secretary of State's office. If you intend for Harry Covey to be the Successor Operator as an individual, please
have your notary delete and initial the deletion of "owner" and "Phil Sheridan Lode Mining" after the words "as"
and "of', respectively, in the "Notary for Successor" section on page 5.
Performance Warranty:
The Division requires the Operation name listed throughout page 1 match the Operaticn name listed on the
Succession of Operators Application (i.e. "Phil Sheridan Lode", rather than "Phil Sheridan Lode Mine").
The Division requires the company name listed in the "Notary for Operator's Acknowledgement" section on page
3 be listed with the Colorado Secretary of State's office. Phil Sheridan Lode Mining is not currently listed with
the Colorado Secretary of State's office. If you intend for Harry Covey to be the Successor Operator as an
individual, please have your notary delete and initial the deletion of "owner" and "Phil Sheridan Lode Mining"
after the words "as" and "of', respectively, in the "Notary for Operator's Acknowledgement" section on page 3.
