Cottony <br />Division Director <br />/ Natural Resource Trustee <br />Re: Canyon Sand & Gravel, Permit No. M-1988-057, Receipt of 110 Constriction Materials <br />Reclamation Permit Amendment Application Package, AM -1 <br />Deaz Mr. Scott: <br />On June 30, 2003, the Division of Minerals and Geology received your the 110 Construction Materials <br />Reclamation Permit Amendment Application package for the Canyon Sand & Gravel, Permit No. M-1988-057. <br />Preliminary review of the information received detemrined that the following items must be received before the <br />Division can consider your application as being submitted and technical review can begin: <br />APPi.iCATTnN FnRMe Page 1, Item 1: The Division requires that the Operator name be the same <br />as the name on the Permit. Page 6, Signature Page: The Division requires that the application be signed <br />in the presence of a Notary Public by the current Permittee. The Division requires that the "Signed and <br />Dated" date and the notary date be the same. <br />F.XAiRTI' R -Site DescrlpNnn rile 6.3.2l~ : You must address Item (a) to the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a <br />minimum, you must include a description of the vegetation and soil characteristics in the area of the <br />proposed operation. The local office of the Natural Resources Conservation Service (NRCS) may <br />provide you with this information as well as recommendations for Exhibit D -Reclamation Plan; <br />F.XAiRiT R _ Rite Deccrlptinn ~ile 63.2 X11: You must address Item (b) to the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a <br />minimum, you must identify any permanent man-made structures within two hundred (200) feet of the <br />affected azea and the owner of each structure. Permanent man-made structlres include houses, barns, <br />fences, above or below ground utilities, imgation ditches, maintained or public roads, bridges, wells, <br />pipelines, water storage impoundments, railroad tracks, cemeteries, and communication antennas, etc. <br />Each structure should be located on Exhibit E -Map; <br />F.XAiRIT R _ Site Degerizntinn (Rule 63.2(~cll: You must address Item (c) to the extent necessary to <br />demonstrate compliance with the applicable performance standard requirements of Rule 3.
